• Enrollment Disclosure Form (included in the application)

• Media Disclosure Form (included in the application)

• An official copy of enrollee's high school or G.E.D. transcript(s) OR official transcripts from institutions attended for college credits

• Resumé(s) of personal, professional and ministry experiences and training, and any related information that may be pertinent for admission consideration
